sponsored by PAK and the Friedman JCC
We will not be offering Moderna at this time,
this is subject to change based on most up to date CDC/FDA Guidelines and supply.

PFIZER - 1st or 2nd dose for ages 12+

PFIZER - Booster available at this time

To qualify for pfizer booster you must:

1.    Answer "Yes" to one of the "Are You??" questions below
2.    Have completed prior 2 dose vaccinations of pfizer at least 6 months ago.
3.    Show proof of your vaccination series

Are you ???:

1.    65 years or older
2.    18 years or older and resident in long term care settings
3.    18 years or older with underlying medical conditions as described in CDC
clinical guidance and in accordance with the CDC's recommendations or
4.    18 years or older with increased risk or getting COVID-19 due to occupational
or institutional exposure, such as frontline essential workers and healthcare workers
as described in and in accordance with CDC recommendations

Please bring the two required forms when you come for your shot.
